INDICATIONS
It is indicated for the treatment of angina pectoris and is used in combination with digitalis and/or diuretics for the treatment of chronic heart failure.
Contraindication
This product is contraindicated in the following patients.
hypersensitivity to nitro compounds or any other ingredients
acute myocardial infarction with low filling pressures
left heart insufficiency with low filling pressures
Shock state
Severe hypotension (systolic blood pressure below 90 mmHg)
Myocardial disease with restriction of intracardiac volume (hypertrophic obstructive cardiomyopathy)
Constrictive pericarditis
Pericardial tamponade
Adverse reaction
Headaches (so-called nitrate headaches) often occur at the beginning of dosing and usually disappear after a few days of continued dosing.
On initial administration or at increased doses, there is often a decrease in blood pressure and/or postural hypotension with reflex pulse rate increase and feelings of weakness and dizziness, sometimes nausea, vomiting, momentary skin redness and warmth, and skin allergic reactions.
In rare cases, a severe decrease in blood pressure with exacerbation of anginal symptoms (paradoxical effect of nitrates) and/or significant paradoxical bradycardia can occur.
Occasionally, collapse and syncope (sudden loss of consciousness) have been reported.
In isolated cases epidermolysis bullosa (inflammatory skin disease) may occur. Resistance and cross-resistance to isosorbide mononitrate with other nitro compounds have been described with continued long-term administration of high doses, and continued use at high doses should be avoided to prevent diminished or lost effectiveness.
Note: Transient arterial blood hypoxia can occur after administration of this product due to relative redistribution of blood flow into areas of the pulmonary alveoli where ventilation is inadequate. Myocardial ischemia can result in patients with coronary artery disease
